Reading all the way through Stu's issue, it appears that both of his
possible proposals call for executing always blocks, then initializers,
then initial blocks.  They differ only in when the other combinational
constructs would run (start or end of slice).  I don't think that really
needs to be specified as long as they run once.

So I agree with the part common to both of Stu's proposals.  I don't
think it is necessary to specify when the other combinational constructs
execute at time 0 (including always_comb).
