[sv-bc] Revised proposal for 74 uploaded

Based on feedback from Dave Rich, Doug Warmke and Brad Pierce,
I have revised the commentary and proposal for erratum 74, and have
uploaded it (look for the "v2" file).

(It actually also covers erratum 75, because the original reported
  issue was split into two errata, 74 and 75).

I have also sent an email to the people who originally raised
the issue, along with the revised proposal, inviting their feedback
(and asking them to cc Dave, Doug, Brad and Matt, since I expect to
be travelling very soon).
