[sv-ec] Forward typedef of a class is only to declare instances of that type

Like other languages, a forward typedef should only be used to declare
an instance of a handle to that type, and nothing else. This issue arose
when someone asked about a forward typedef the base of an extended
class. Section 7.24 should add that restriction. Entered as mantis 1500
