[sv-bc] SV 2009 configs -- index expressions in hierarchical names

According to 33.4.3, "Index expressions in a hierarchical name shall only refer to literals or localparams of the configuration." But Syntax 33-4 doesn't allow index expressions in hierarchical names. As mentioned in http://www.eda.org/svdb/view.php?id=986 , "The LRM does not mention about configuring instance arrays and generated instantiations. Also, as per the BNF (Syntax 13-6), it is illegal to configure an element of an instance array/generate block because 'inst_name' can contain 'instance_identifier', only."

This restriction doesn't seem to be talking about the param_expression in the named_parameter_assignment, because another restriction in the same list is, "Hierarchical references cannot include scopes of generate or array of instances."
