Small point in "Section 18.10 Hierarchical Names":
"Hierarchical names ... consist of instance names separated by
periods, where an instance name can be an array element."
Presumably "instance names" here also includes module names that are
implicitly instantiated in $root? Should we add a sentence to say
this?
