[sv-bc] New guidelines for P1800 mantis proposals

To all P1800 Technical Committees,

During the IEEE review process for the set of changes made to the 1800-2009
version of the LRM there was some push-back from the IEEE.

There are a couple of things that can be done to ensure that this process
goes more smoothly this time around. The following new policies are now in
effect.

    1. Mantis proposals must be provided in the pdf format

       Mantis proposals that have been approved by the Technical Committees
       need to be provided in pdf format. This ensures that everyone will be
       seeing the same set of changes. We have seen cases in the past where
       some html files don't display the same way in different browsers.

    2. Mantis proposals shall have a title (showing the mantis number)

       Each mantis proposal will contain a title at the top of the proposal.
       This title will clearly identify which mantis item it is associated with.
       This makes it easy to determine where one proposal begins and another
       ends when all of the approved proposals are rolled up into a single pdf
       file. The title should be given in a large bold font.

       An example of a title would be: Mantis 2683
